You can fax or mail your orders to be paid via Visa, Mastercard, Discover, or PayPal. If paying by check or money order simply mail to the address below. You may print out this order form by clicking the button below. Your billing address is that shown on your credit card statement. Upon receipt of your order, we will send you a confirmation email invoice for your records. If we will be shipping to a California address there is a 8.25% sales tax on the products. There is no sales tax for shipments to other states. The shipping charge should be added to your product and sales tax total. The shipping charge is $2.90 for orders with product total of $100 or more. The shipping charge is $4.90 for orders with product total of less than $100. There is a $25 minimum order size please.
